କୁପୋଷଣ (ଅପପୁଷ୍ଟି ବା ପୁଷ୍ଟିମାନ୍ଦ୍ୟ ନାମରେ ମଧ୍ୟ ଜଣା, ଇଂରାଜୀ: Malnutrition ବା malnourishment) ଏକ ସ୍ୱାସ୍ଥ୍ୟ ସମସ୍ୟା ଅବସ୍ଥା ଯାହା ଯଥେଷ୍ଟ ପୋଷକ ନ ଥିବା କିମ୍ବା ମାତ୍ରାଧିକ ପୋଷକ ଥିବା ଖାଦ୍ୟ ଖାଇବା ଫଳରେ ସୃଷ୍ଟି ହୁଏ ।[୧][୨] ଖାଦ୍ୟରେ ଅନେକ ଧରଣର ପୋଷକ ଥାଏ । ଶ୍ୱେତସାର, ପୁଷ୍ଟିସାର, ଜୀବସାର, ଖାଦ୍ୟ ଲବଣ ଓ କ୍ୟାଲୋରିକୁ ପୋଷକ କୁହାଯାଏ । [୨] କୁପୋଷଣ ଅଧିକାଂଶ ସମୟରେ ଅପ‌ପୁଷ୍ଟିକୁ ବୁଝାଏ ଯେଉଁଥିରେ ଯଥେଷ୍ଟ କ୍ୟାଲୋରି, ପୁଷ୍ଟିସାର କିମ୍ବା ମାଇକ୍ରୋପୋଷକ ନ ଥାଏ ତ‌ଥା ଅଧିକ ପୋଷଣକୁ (overnutrition) ମଧ୍ୟ ବୁଝାଏ । [୩][୪] ଗର୍ଭାବସ୍ଥା ଓ ଦୁଇ ବର୍ଷ ବୟସରୁ କମ ଥିବା ପିଲାଙ୍କର ଅପ‌ପୁଷ୍ଟି ହେଲେ ଶାରୀରିକ ତ‌ଥା ମାନସିକ ବୃଦ୍ଧି ହୋଇପାରେ ନାହିଁ । [୨] ଅତ୍ୟଧିକ ପୋଷକ ଅଭାବ ଯେପରିକି ଉପବାସ ଅବସ୍ଥା ହେଲେ ଉଚ୍ଚତା କମ୍ ହୁଏ, ପତଳା ଶରୀର ହୁଏ, ଶକ୍ତିସ୍ତର କମିଯାଏ ଓ ପେଟ ତ‌ଥା ଗୋଡ଼ ଫୁଲିଯାଏ । [୨][୩] ଏଥିରେ ଲୋକର ଅଧିକ ରୋଗ ସଂକ୍ରମଣ ହୁଏ ଓ ଦେହ ଥଣ୍ଡା ରହେ । ମାଇକ୍ରୋପୋଷକ ଅଭାବ ଲକ୍ଷଣ ତାହାର ପ୍ରକାର ଅନୁସାରେ ହୁଏ । [୩]

କୁପୋଷଣ ଏକ ଏପରି ଅବସ୍ଥା ଯାହା ଖାଉଥିବା ଖାଦ୍ୟରେ କେତେକ ପୋଷକର ଅଭାବ, ଆଧିକ୍ୟ, କିମ୍ବା ଅସନ୍ତୁଳିତ ଥିବା ଯୋଗୁ ଉପୁଜେ ।[୫][୬] ଏହାକୁ ଇଂରାଜୀରେ ମାଲ୍‌ନ୍ୟୁଟ୍ରିସନ୍ (Malnutrition) ବା ମାଲନରିସମେଣ୍ଟ (malnourishment) କହନ୍ତି । ଖାଦ୍ୟରେ ଅନେକ ଧରଣର ପୋଷକ ଥାଏ ଯଥା ଶ୍ୱେତସାର, ପୁଷ୍ଟିସାର, ଜୀବସାର, ଖାଦ୍ୟ ଲବଣ ଓ କ୍ୟାଲୋରିକୁ ପୋଷକ କୁହାଯାଏ । [୨] ଓଡ଼ିଆରେ ସାଧାରଣତଃ ଭୁଲ୍ କ୍ରମେ ଏହାକୁ ଖାଦ୍ୟାଭାବ ବୋଲି ଧରାଯାଏ । ତେଣୁ ପୋଷକର ପ୍ରାବଲ୍ୟ ବା ଅଭାବ ଭେଦରେ ବିଭିନ୍ନ ପ୍ରକାରର ପୋଷକ ରୋଗ ହୁଏ । ପୃଥିବୀର ଅଧିକାଂଶ ସ୍ଥାନରେ ଖାଦ୍ୟାଭାବ ତଥା କ୍ୟାଲୋରି ଓ ପ୍ରୋଟିନ ଅଭାବ ଜନିତ ଅପପୁଷ୍ଟି ଦେଖାଯାଏ[୭] । ସ୍ୱଳ୍ପ ବିକଷିତ ରାଷ୍ଟ୍ରମାନଙ୍କରେ ଅପପୁଷ୍ଟି ଦେଖାଯାଏ । କାରଖାନା ପ୍ରବଣ ଦେଶମାନଙ୍କରେ ମଧ୍ୟ ଏହା ଦେଖାଯାଏ । ଧନୀ ଦେଶମାନଙ୍କରେ ଅସ୍ୱାସ୍ଥ୍ୟକର ଖାଦ୍ୟ, ଅଧିକ ଶକ୍ତି ସମ୍ପର୍ଣ୍ଣ ଚର୍ବି ଓ ଶ୍ୱେତସାର ଖାଦ୍ୟ ଖାଇବା ଯୋଗୁ ହୁଏ । ପୃଥୁଳତା ଗୋଟିଏ ଜନ ସ୍ୱାସ୍ଥ୍ୟ ସମସ୍ୟା ରୂପେ ଉଭା ହେଲାଣି[୮]

ଉତ୍ତମ ମାନର ଖାଦ୍ୟ ଖାଇବାକୁ ନ ମିଳିଲେ ଅପ‌ପୁଷ୍ଟି ହୁଏ । [୯] ଖାଦ୍ୟର ଅତ୍ୟଧିକ ଦାମ୍ ଓ ଦାରିଦ୍ର୍ୟ ଯୋଗୁ ଏହା ସମ୍ଭବ ହୁଏ । [୨][୯] ସ୍ତନ୍ୟପାନ ନ କରିବା, ଗ୍ୟାସ୍ଟ୍ରୋଏଣ୍ଟେରାଇଟିସ, ନିମୋନିଆ, ମ୍ୟାଲେରିଆମିଳିମିଳା ଭଳି ସଂକ୍ରାମକ ରୋଗ ହେଲେ ପୋଷଣ ଆବଶ୍ୟକତା ବଢ଼େ ଓ ଫଳତଃ ଏହି କୁପୋଷଣ କରିବାରେ ସାହାଯ୍ୟ କରନ୍ତି । [୯] ପୁଷ୍ଟିସାର-କୁପୋଷଣ ଓ ଖାଦ୍ୟାଭାବ, ଏହି ଦୁଇ ପ୍ରକାର ଅପ‌ପୁଷ୍ଟି ଦେଖାଯାଏ । [୪] ପୁଷ୍ଟିସାର କୁପୋଷଣ ଦୁଇ ଗମ୍ଭୀର ପ୍ରକାର ଥାଏ: ମାରାସ୍‌ମସ୍ (marasmus), ଯେଉଁଥିରେ କ୍ୟାଲୋରି ଓ ପୁଷ୍ଟିସାର ଅଭାବ ଥାଏ ଓ କ୍ୱାସିଓର୍କର (kwashiorkor) ଯେଉଁଥିରେ ପୁଷ୍ଟିସାର ଅଭାବ ଥାଏ । [୩] ସାଧାରଣ ମାଇକ୍ରୋନ୍ୟୁଟ୍ରିଏଣ୍ଟ ମଧ୍ୟରେ ଲୌହ, ଆୟୋଡିନ ଓ ଭିଟାମିନ ଏ ଅଭାବ ଅଭାବ ମୂଖ୍ୟସ୍ଥାନରେ ଥାଏ ।[୩] ଗର୍ଭାବସ୍ଥାରେ ଅଧିକ ଚାହିଦା ଯୋଗୁ ପୋଷକ ନିଅଣ୍ଟ ଅଧିକ ପଡ଼େ ।[୧୦] କେତେକ ବିକାଶଶୀଳ ଦେଶମାନଙ୍କରେ ଅପ‌ପୁଷ୍ଟି ସାଥିରେ କେତେକ ଗୋଷ୍ଠୀରେ ମୋଟାପଣ ମଧ୍ୟ ଦେଖାଯାଉଛି । [୧୧] କୁପୋଷଣର ଅନ୍ୟ କାରଣମାନଙ୍କ ମଧ୍ୟରେ ଆନୋରେକ୍ସିଆ ନର୍ଭୋସା (anorexia nervosa) ଓ ବାରିଆଟ୍ରିକ ଅପରେଶନ (bariatric surgery) ଅନ୍ୟତମ । [୧୨][୧୩] ବୟସ୍କମାନଙ୍କ କ୍ଷେତ୍ରରେ ଶାରୀରିକ, ମନସ୍ତାତ୍ତ୍ୱିକ ଓ ସାମାଜିକ ପ୍ରଭାବ ଯୋଗୁ ଅଧିକ କୁପୋଷଣ ଦେଖାଯାଏ । [୧୪]

ବିକାଶ ଯୋଜନାରେ ପୋଷକ ଉନ୍ନତି ପ୍ରଚେଷ୍ଟା ଅତି ଫଳପ୍ରଦ ବ୍ୟବସ୍ଥା ଅଟେ । [୧୫] ସ୍ତନ୍ୟପାନ କରାଇଲେ ଶିଶୁ କୁପୋଷଣ ଓ ମୃତ୍ୟୁ ହାର କମାଏ, [୨] ଓ ପ୍ରଚେଷ୍ଟାଦ୍ୱାରା ଉନ୍ନତି ହାର ବଢ଼େ । [୧୬] ୬ ମାସରୁ ୨ ବର୍ଷ ବୟସ ମଧ୍ୟରେ ସ୍ତନ୍ୟପାନ ସ‌ହ ଅନ୍ୟ ଖାଦ୍ୟ ଦେଲେ ଫଳାଫଳ ଉନ୍ନତ ହୁଏ । [୧୬] ବିକାଶଶୀଳ ଦେଶମାନଙ୍କରେ ଅନେକ ମାଇକ୍ରୋନ୍ୟୁଟ୍ରିଏଣ୍ଟ ଦେଇ ଖାଦ୍ୟପୂର୍ତ୍ତି (supplementation) କଲେ ଉପକାରର ପ୍ରମାଣ ମିଳୁଛି । [୧୬] ଅଭାବୀ ଲୋକଙ୍କୁ ଅର୍ଥ ସାହାଯ୍ୟ କରିବାଦ୍ୱାରା ସେମାନେ ସ୍ଥାନୀୟ ବଜାରରୁ ଖାଦ୍ୟ କିଣି ଖାଇବା ଫଳପ୍ରଦ ହୁଏ ।[୧୫][୧୭] କେବଳ ସ୍କୁଲରେ ଖାଦ୍ୟ ଖୁଏଇବା ଯଥେଷ୍ଟ ନୁହେଁ । [୧୫] ଅଧିକାଂଶ ସମୟରେ ଗମ୍ଭୀର କୁପୋଷଣ ରୋଗୀକୁ ତାଙ୍କର ନିଜ ଘରେ ପ୍ରସ୍ତୁତ-ଖାଦ୍ୟ ଦେବା ସମ୍ଭବ । [୧୬] ଅବସ୍ଥା ଅତି ଗମ୍ଭୀର ଥିଲେ ଡାକ୍ତରଖାନାରେ ଆଡମିଶନ ଆବଶ୍ୟକ ହୁଏ । [୧୬] ନିମ୍ନ ରକ୍ତ ଶର୍କରା, କମ୍ ଶରୀର ଉତ୍ତାପ, ନିର୍ଜଳନର ଚିକିତ୍ସା ସ‌ହ କ୍ରମାନ୍ୱୟ ଖାଦ୍ୟ ଦେବା ବ୍ୟବସ୍ଥା କରାଯାଏ । [୧୬][୧୮] ସଂକ୍ରମଣ ସମ୍ଭାବନା ଥିବାରୁ ନିୟମିତ ଆଣ୍ଟିବାୟୋଟିକ୍ ଦେବାକୁ ପରାମର୍ଶ ଦିଆଯାଏ । [୧୮] ଦୀର୍ଘକାଳୀନ ବ୍ୟବସ୍ଥାରେ କୃଷି ବ୍ୟବସ୍ଥା ଆବଶ୍ୟକ, [୧୯] ଯାହାଦ୍ୱାରା ଦାରିଦ୍ର୍ୟ ଦୂରୀକରଣ, ଉନ୍ନତ ପରିମଳ ବ୍ୟବସ୍ଥା ଓ ମହିଳା ସଶକ୍ତିକରଣ ହୋଇପାରିବ । [୧୫]

ସନ ୨୦୧୦ ସୁଦ୍ଧା ପୃଥିବୀରେ ୯୨୫ ନିୟୁତ ଅପ‌ପୁଷ୍ଟି ରୋଗୀ ଥିଲେ ଯାହା ୧୯୯୦ ସନ ଅପେକ୍ଷା ୮୦ ନିୟୁତ ଅଧିକ ଥିଲା ।[୨୦][୨୧] ଆଉ ଏକ ବିଲିଅନ ଲୋକ ଭିଟାମିନ ଓ ଖାଦ୍ୟ ଲବଣ ଅଭାବରେ ପୀଡ଼ିତ ଅଛନ୍ତି । [୧୫] ସନ ୨୦୧୦ରେ ପୁଷ୍ଟିସାର-ଶକ୍ତି କୁପୋଷଣ ମୃତ୍ୟୁ ସଂଖ୍ୟା ୬୦୦,୦୦୦ ଥିଲା ଯାହା ସନ ୧୯୯୦ର ଉଲ୍ଲିଖିତ ସଂଖ୍ୟା ଥିଲା ୮୮୩,୦୦୦ । [୨୨] ଅନ୍ୟାନ୍ୟ ପୋଷକ ଅଭାବ ନାମ ଆୟୋଡିନ ଅଭାବ, ଲୌହାଭାବ ଆନିମିଆ ଯୋଗୁ ୮୪,୦୦୦ ମୃତ୍ୟୁ ହୋଇଥିଲା । [୨୨] ଅନୁପୋଷଣ (undernutrition) ଯୋଗୁ ସନ ୨୦୧୦ରେ ୧.୪% ବିକଳାଙ୍ଗ ନିୟନ୍ତ୍ରିତ ଜୀବନବର୍ଷ (disability adjusted life years) ଥିଲା । [୧୫][୨୩] ପିଲାଙ୍କ ମୃତ୍ୟୁ ସଂଖ୍ୟାର ପ୍ରାୟ ଏକ ତୃତୀୟାଂଶ ଅନୁପୋଷଣ ଯୋଗୁ ହେବା ବିଶ୍ୱାସ କରାଯା‌ଥିଲା କିନ୍ତୁ ସମସ୍ତ ରୋଗୀଙ୍କୁ ଏହି ଦଳରେ ନିଆଯାଇ ନ ଥିଲା । [୯] ସନ ୨୦୧୦ର ହିସାବ ଅନୁସାରେ ଏହି ରୋଗ ଯୋଗୁ ମହିଳା ଓ ପିଲାଙ୍କ ମୃତ୍ୟୁ ୧.୫ ନିୟୁତ ଥିଲା, [୨୪] ଯଦିଓ କେତେକ ହିସାବ ଅନୁସାରେ ଏହା ୩ ନିୟୁତରୁ ଅଧିକ ହେବା ସମ୍ଭାବନା ଅଛି । [୧୬] ଏହି ରୋଗ ଯୋଗୁ ଅଧିକ ୧୬୫ ନିୟୁତ ପିଲାଙ୍କର ବାମନ-ବିକାଶ ଥିଲା । [୧୬] ବିକାଶଶୀଳ ଦେଶମାନଙ୍କରେ ଅନୁପୋଷଣ ସାଧାରଣତଃ ଦେଖାଯାଏ । [୨୫]

ଡବ୍ଲ୍ୟୁ.ଏଚ୍.ଓ. ରିପୋର୍ଟ ଅନୁସାରେ ଭୋକ ଓ ତତ୍ ସମ୍ବନ୍ଧୀୟ ଅପପୁଷ୍ଟି ବର୍ତ୍ତମାନ ଏକକ ଜନ ସ୍ୱାସ୍ଥ୍ୟ ସଙ୍କଟ ହେଲାଣି[୨୬][୨୬][୨୭] । ଉପଯୁକ୍ତ ପୋଷକ ପାଇଲେ ଏହାର ସମାଧାନ ହେବ । ଅପପୁଷ୍ଟିର କାରଣ ଅନୁସାରେ ଉପଯୁକ୍ତ ପୋଷକ ଦେଇ ପାରିଲେ ଅର୍ଥର ଉପଯୁକ୍ତ ବିନିଯୋଗ ହୋଇ ପାରିବ[୨୮] । ଅତ୍ୟଧିକ ଅଭାବ ଥିଲେ ଆପାତ କାଳୀନ ଭିତ୍ତିରେ ଉନ୍ନତ ମାଇକ୍ରୋପୋଷକ (Micronutrient) ସ୍ୟାଚେଟ (Sachet) ଭିତରେ ବା ଖାଦ୍ୟ ସହ ଦିଆଯାଏ[୨୯][୩୦] [୩୧] । ଦେହରେ ଅତି ସ୍ୱଳ୍ପ ମାତ୍ରାରେ ଆବଶ୍ୟକ ହେଉଥିବା ଖଣିଜ ପଦାର୍ଥକୁ ମାଇକ୍ରୋପୋଷକ କୁହାଯାଏ । ଆପାତ ସ୍ଥିତିରେ ଅତ୍ୟଧିକ ଅପପୁଷ୍ଟି ହେଲେ ପୂର୍ବ ପ୍ରସ୍ତୁତ ଉପଯୁକ୍ତ ଖାଦ୍ୟ ଦେବାଦ୍ୱାରା ଓଜନ ବଢିବା ଦେଖା ଯାଉଥିବାରୁ ଡବ୍ଲ୍ୟୁ.ଏଚ୍.ଓ., ୟୁନିସେଫ୍ ଓ ୟୁ.ଏନ୍.ଫୁଡ ପ୍ରୋଗ୍ରାମ ଏହାକୁ ସୁପାରିଶ କରିଛନ୍ତି । ଦୁର୍ଭିକ୍ଷ ସମୟରେ ଅର୍ଥ (Cash) ବା କ୍ୟାସ ଭାଉଚର୍ (Cash voucher) ଲୋକଙ୍କୁ ଦେଲେ ସେମାନେ ସ୍ଥାନୀୟ ଚାଷୀଙ୍କଠାରୁ ଖାଦ୍ୟ କିଣି ଖାଇଲେ ଦାନରୁ ମିଳିଥିବା ଖାଦ୍ୟଠାରୁ ଭଲ ହେବ । ଉଡାଜାହାଜରୁ ଖାଦ୍ୟ ତଳକୁ ପକାଇଲେ ତଳେ ଥିବା ଲୋକମାନେ ଆଘାତପ୍ରାପ୍ତ ହେବା ଆଶଙ୍କା ଥାଏ[୩୨][୩୩]

ଦୀର୍ଘ ସୁତ୍ରୀ କାର୍ଯ୍ୟକ୍ରମ ହାତକୁ ନେଇ ଉନ୍ନତ କୃଷି ପ୍ରଣାଳିରେ ଚାଷ କଲେ ଭବିଷ୍ୟତରେ ଖାଦ୍ୟାଭାବ ଦୂର ହେବ, କିନ୍ତୁ ବିଯୁକ୍ତାତ୍ମକ ଫଳ ପ୍ରତି ସତର୍କ ରହିବାକୁ ହେବ[୧୯] । ଆଧୁନିକ ସ୍ଥିତିରେ ଚାଷୀକୁ ସବୁ ପ୍ରକାର ସାହାଯ୍ୟ କରିବାକୁ ଚେଷ୍ଟା କରାଯାଉଛି[୩୪] । ବର୍ଲ୍ଡ ବ୍ୟାଙ୍କ ଚାଷୀଙ୍କୁ ସବସିଡି ଦେବାରେ କିଛି ବାଧକ ରଖିଛି, ସାର ବ୍ୟବହାରରେ[୩୫] ମଧ୍ୟ ପରିବେଶ କ୍ଷତିଗ୍ରସ୍ତ ଓ ମାନବ ସ୍ୱାସ୍ଥ୍ୟ ହାନୀ[୩୬] ହେବାର ସମ୍ଭାବନା ଅଛି ଓ ଅନେକ ସୋସାଇଟି ଏହାକୁ ବିରୋଧ କରୁଛନ୍ତି[୩୭]
ମହିଳା, ପିଲା ଓ ବୟସ୍କ ଲୋକମାନଙ୍କର ଅପପୁଷ୍ଟି ଅଧିକ ଧ୍ୟାନ ଆକର୍ଷଣକାରୀ ଅଟେ । ସ୍ତନ୍ୟ ପାନ ଓ ଗର୍ଭ ଯୋଗୁ ମହିଳାମାନେ ଅଧିକ ପୋଷକ ଆବଶ୍ୟକ କରନ୍ତି । ଗର୍ଭଧାରୀ ମାତାର ଅପପୁଷ୍ଟି ଥିଲେ ଗର୍ଭସ୍ଥ ଶିଶୁ ଉପରେ ମଧ୍ୟ ଅପପୁଷ୍ଟି ପ୍ରଭାବ ପଡେ । ସ୍ତନ୍ୟ ପାନଦ୍ୱାରା ନୂତନ ଶିଶୁର କୁପୋଷଣ କମି ଯାଏ । ମାତା ଶିକ୍ଷିତ ହେଲେ ତାହାର ପ୍ରଭାବ ପଡ଼ିବ[୩୮] । ବୟଷ୍କମାନଙ୍କର କମ୍ ଭୋକ, ଚୋବେଇବା ତଥା ଗିଳିବା ଅସୁବିଧା ଓ କମ ଏନର୍ଜି ଯୋଗୁ ଅପପୁଷ୍ଟି ସଙ୍କଟ ଅଧିକ ରହେ । ଏମାନଙ୍କର ଅଧିକ ଯତ୍ନ ଆବଶ୍ୟକ ।
